Overview
Social Worker, Medical Social (Travel - Temporary Assignment) | Lawrenceville, Georgia | Start May 18, 2026 | 8 weeks assignment | 40 guaranteed hours per week | Schedule: Monday–Friday, 8:00 a.m.–4:30 p.m. | Acute hospital setting. This travel position provides an opportunity to contribute to patient and family care in a hospital-based social work team while on a 40-hour week with guaranteed hours.

Responsibilities
- Conduct psychosocial assessments for patients and families to identify barriers to care and discharge planning needs
- Develop, implement, and monitor patient discharge plans in collaboration with the multidisciplinary team
- Provide counseling, coping strategies, and resource referrals to patients and families navigating medical issues
- Coordinate with medical staff, case management, social work peers, and community resources to ensure safe transitions to home or alternate care settings
- Maintain accurate, timely documentation in patient records and ensure compliance with hospital policies and regulatory requirements
- Advocate for patient rights and preferences, supporting culturally competent and patient-centered care

Requirements
- Active Georgia LCSW license (GA LCSW)
- Minimum of 2 years of experience in medical social work or a closely related specialty
- Acute hospital experience required
- Ability to work effectively within a multidisciplinary team in a high-acuity setting
- Eligibility to work in the United States and willingness to participate in a travel assignment
