Job Title: Licensed Master Social Worker (LMSW)

Location: North Kansas City, Missouri

Salary: $65,000-$72,500

Shift: Full-time, Monday through Friday, 8:00 AM to 5:00 PM



Job Overview:
We are seeking a Licensed Master Social Worker (LMSW) to join its Home Health and Hospice team serving both Kansas and Missouri locations. This is a full-time, flexible role focused on providing hospice and home health social work services to adult and geriatric patients in their homes. The LMSW plays a critical role in supporting patients and families through psychosocial assessments, care planning, education, and emotional support while collaborating closely with interdisciplinary care teams. This position offers long-term stability with a well-established agency and a strong emphasis on holistic, patient-centered care.

Coverage Details:

  • Coverage Area: North Kansas City, including Liberty, Platte City, Excelsior Springs, Blue Springs, and Independence
  • Average Daily Mileage: Approximately 50 to 100 miles per day, typically 50–70 miles
  • Mileage Reimbursement: $0.45 per mile, excluding mileage to and from home

Key Responsibilities:

  • Provide hospice and home health social work services through in-home patient visits
  • Conduct psychosocial assessments and develop care plans tailored to patient and family needs
  • Support patients and families with counseling, education, and resource coordination
  • Collaborate with interdisciplinary team members to ensure comprehensive care delivery
  • Assist patients and families with advance care planning and end-of-life decision-making
  • Document assessments, interventions, and care plans using electronic charting systems
  • Monitor patient needs and communicate changes to the care team
  • Promote holistic, non-pharmacologic approaches when appropriate, addressing nutrition, lifestyle, and psychosocial factors
  • Maintain compliance with all federal, state, and organizational regulations

Required Qualifications:

  • Master of Social Work (MSW) degree from an accredited program
  • Active LMSW license in Missouri and/or Kansas, as applicable
  • Minimum of 1 year of experience working in a healthcare setting, hospice experience preferred
  • Current CPR certification
  • Valid driver's license and active auto insurance
  • U.S. citizenship or valid green card
  • Strong communication, assessment, and time management skills
  • Ability to work independently in a home-based care environment
