Job Summary
Full-Time Housing Manager
Jonathan’s Path is a Tennessee-based nonprofit that bridges critical gaps in foster care by providing rapid, barrier-free access to comprehensive support through the Path Line and a readiness-based progressive housing model—ensuring teens in and aging out of foster care receive the stability, practical resources, and consistent adult relationships they need to heal, grow, and truly thrive as we become the “people who stay” in their lives.
Job Responsibilities
Core Responsibilities
Progressive Housing Leadership
- Serve as lead coordinator for all youth placed within JP’s housing continuum
- Support the development and implementation of new Path Home locations
- Ensure program fidelity across Path Home, Path Forward, and Path Beyond models
- Collaborate with leadership to refine housing processes, policies, and best practices
Path Home Oversight
- Provide direct support and supervision to live-in caregivers
- Conduct regular home visits and safety checks
- Support caregivers with conflict resolution, youth transitions, and care planning
- Ensure compliance with all licensing and program requirements
Youth Case Management
- Provide individualized case management for youth in JP housing
- Conduct minimum monthly check-ins (in-person preferred) with each housed youth
- Assist youth with goal planning related to housing stability, employment, education, and life skills
- Support readiness-based transitions between housing tiers
- Coordinate with DCS, Bright Futures, landlords, and community partners as needed
Housing Navigation & Placement
- Review and respond to housing submissions received through JP’s intake form
- Assess youth eligibility and readiness for placement
- Identify and secure safe, affordable housing options
- Build and maintain relationships with landlords and property managers
- Assist youth with lease signing, move-in coordination, and housing stabilization
Data & Documentation
- Maintain accurate and up-to-date records in JP’s CRM system
- Document case notes, housing status, contacts, and outcomes in real time
- Track housing stability metrics and provide monthly reporting
- Ensure compliance with grant reporting and funding documentation requirements
Relationship & System Navigation
- Build trust with youth navigating complex systems
- Help youth overcome systemic barriers to housing access
- Serve as an advocate when navigating housing denials or placement challenges
Qualifications
Qualifications
Required:
- Bachelor’s degree in Social Work, Human Services, Psychology, or related field (or equivalent experience)
- 3+ years experience working with vulnerable populations (foster youth strongly preferred)
- Experience in housing navigation, case management, or residential program oversight
- Strong understanding of trauma-informed care
- Exceptional documentation and organizational skills
- Ability to manage multiple youth cases simultaneously
- Valid driver’s license and reliable transportation
Preferred:
- Experience working with foster care systems or DCS
- Experience supervising residential or live-in staff
- Familiarity with CRM systems (Salesforce, HubSpot, or similar)
- Knowledge of Tennessee housing resources

Work Environment
- Hybrid work schedule: 40% remote, 60% in-office or in the field.
- A minimum of forty hours per week is expected. Schedule is typically Monday-Friday but must be available for evening and weekend meetings and events.
